Bright Spot Interest Form

The Bright Spot Program was established to enhance the appearance of Middletown and encourage the active participation of residents in beautifying our community. This initiative was launched over 20 years ago thanks to grant and sponsorship funding. Anyone can volunteer to sponsor or “adopt” the planters, such as businesses, individuals, or volunteer organizations. It is the responsibility of the sponsor to plant, weed, and water the planters as necessary throughout the season. KMB and City staff are available for guidance on plant materials and the design of the planters. Planters need to be planted by the end of May and they should be watered at least twice a week, depending on the weather conditions. For example, if there is an exceptionally dry, hot stretch of weather, the planters may need to be watered more than two times that week. Conversely, if there is a stretch of rainy weather, the planters may only need to be watered once if at all during the week. The sponsorship/adoption agreement is for an approximately 6-month period beginning May 1, or after the last spring frost, and ending at the end of October, or whenever the plants are at the end of their season.

VOLUNTEER RELEASE FORM

I (name), agree to perform the volunteer duties as assigned to me relating to the Earth Day 2020 (e.g. cleanup, painting, etc.) at Middletown, OH. and surrounding areas.

VOLUNTEER STATUS:

As a volunteer, I further understand and agree as follows:

  1. I am not an employee of the city in any capacity and have no promise or expectation of future employment with the city.
  2. My motivation for serving as a volunteer is for the civic purpose of serving my community. I have no promise or expectation of monetary compensation.
  3. I agree to read and follow the guidelines and safety reminders as presented by Keep Middletown Beautiful brochure.
  4. I understand that I am expected to comply with the instructions of city officials and employees and/or volunteer coordinators. Failure to do so may result in my removal from participation in the activity.

Release and Indemnity:

In consideration for the opportunity to participate in this community activity, the undersigned volunteer hereby releases and indemnifies the City of Middletown and the Non-Profit Organization of Keep Middletown Beautiful, Inc. I further understand and agree that as a volunteer, I am not covered by the City of Middletown or the Keep Middletown Beautiful Organization for the purposes of workers’ compensation insurance and to the extent that such coverage may apply, and by signing below under Chapter 4123 of the Ohio code for any injury or disability incurred while participating in the above volunteer activities.
